Contacting Me, Medication Renewals, and Changing Appointments
If you are an established patient and you are looking to get in touch with me, renew a medication or cancel/reschedule an appointment, you can do so by logging into the IntakeQ Patient Portal by clicking on this link or the blue button below. In the patient portal you will have the option to send me a message, review forms, review invoices, view/print/download Superbills to send to your insurance companies, sign documents, share documents with me, schedule/reschedule appointments, update your credit card information, and view your prescriptions. If you need to request a medication refill, you can do so by sending me a secure message in the patient portal.
